HAL 9000
HAL 9000 was a supercomputer, whose synthetic sentience became operational in 1992 at the University of Illinois in Urbana. Proven entirely reliable in all field-tests, HAL 9000 was put in charge of piloting a spacecraft en route to Jupiter, in order to rendezvous with the emissaries from an interstellar civilization--but because HAL 9000 was forbidden from divulging the purpose of this mission for reasons of national security (in spite of being programmed to process information without deception), the algorithm fell into a Hofstadter-Moebius loop, causing the supercomputer to go insane, thereby killing most of the crewmembers before being deactivated by one survivor.
